This item is in a cave guarded by a runic puzzle that kicks you around a bit. The runes will teleport you outside the cave, a ways in the air - so you take falling damage - and the power lines throw you backwards. Don't keep running onto the runes or you'll eventually die from fall damage.

A couple easy ways to get in:

mount up and just run along the cave wall to the right
walk backwards through the first ley line (it then throws you backwards)
use blink, levitate, or disengage (backwards) if you have it

If you ever figure out a method to turn off the runes or ley lines, please comment. I tried and never did figure it out.
