Can confirm this item can appear in the Proving Grounds as of 02/17/17. Screenshot has been submitted for proof. If interested, my methodology was as follows:

1. Select the "Blighted Rune Weapon" and "Castigator" talents to quickly and reliably build up to at least 6 festering wounds.
2. Enter the Proving Grounds and select one of the trials (I used bronze damage)
3. Immediately after selecting one, cast Army of the Dead
4. Activate Blighted Rune Weapon and stand where the mob will spawn
5. Spam Festering Strike until you have at least 6 wounds, then cast Apocalypse
6. Immediately Wraith Walk back to Trial Master Rotun to yield and repeat this process.

A Note About Army of the Dead vs. Apocalypse: There appears to be some uncertainty as to whether "Army of the Dead" can spawn the Orc that signifies the spawning of the journal. While I personally assume only the Apocalypse ability can spawn it (Artifact Appearance unlocked by Artifact Ability just makes sense to me I suppose), an easy way for data to be collected about this is to ensure Army of the Dead is cast as soon as the trial starts. This is because your chatbox can somewhat reliably provide a time-stamp of the event occurring, thanks to the fact that the spawned orc speaks into your chatbox like so: "Army of the Dead says: Why have I been brought back to fight for you?"

Trial Master Rotun speaks twice during the farming process (of the bronze damage trial), once to signify the beginning of the trial and once to announce the spawning of the first mob roughly seven seconds later. Army of the Dead has a four second channel time, so all of your Army of the Dead spawns should have spawned by the time the first mob appears. This means if the message from the special Army of the Dead appears before the message signifying the spawning of the first enemy, then Army of the Dead summoned him. If it appears afterward (like it did for me), then the Apocalypse ability spawned him.

I mention this only because it could help determine whether Army of the Dead can summon the special ghoul or not.

I can confirm, that you don't need to find this book by yourself to obtain the hidden skin. I used a book left by another unholy Deathknight at the Flightmaster in Bradensbrook.

Unshure if it would work when reading a strangers book, I made my way to Icecrown and up to the laboratory. Can't describe how happy i was when I saw the trap door.

Found it during the Assault on Broken Shore scenario, during stage 2 Vanguard of the Assault, while fighting a group of mobs. Position 51,69. Didn't know this was possible, was more like a coincidence.
Also noted a line from the army of the dead while being summoned "Why have I been brought back to fight for you?" which sounded strange to me as I never noticed it before.

Just to clear things up, when you have read the book you can go to ICC, when you defeated professor putricide you will be able to enter a hidden trapdoor once inside you'll find stitchwork who drops the hidden appearance.

Found on Argus, near Prophet's Reflection. Did a wq where I had to find a demon spy, and after killing it the book dropped. Came here to see what it's for. Actually kind of wish I had thought more myself and figured to go to Putricide's room in ICC..

Confirmation that the item still spawned for me on an alt. Lvl. 117 Horde, around 0500 Eastern time Americas on 12/10/2018, while questing in Zanchul. And the realm was Kael'thas.

Can confirm this still drops in 8.1. I just had it drop off an Apocalypse summon in Drustvar.
I did not have the artifact equipped or in my bags. However, I had it in my bank.

Since the Apocalypse spell still exist you can get the hidden skin. The journal dropped for me when I was soloing Black Rook Hold at level 115. I was not wearing the actual artifact weapon but I had my current weapon transmoged into the green pvp skin.

This just dropped for me after doing the Crown Chem dungeon for Big Love Rocket. I had no idea what it was and I of course instantly clicked it because at this point I assume any lag or anything out of the norm is a sign I'm about to get Big Love Rocket lol.

This is the trigger for the unholy hidden artifact appearance. Your location does not affect getting this nor does having the Unholy artifact weapon equipped. See 뼈 수확자의 갈고리 for the details on how to trigger the Journal to appear and the macro to determine if you may have already triggered it. (For the record, mine dropped while doing an Island Expedition.)

I literally found this lying on the ground on the stairs at Plunder Harbor while doing the introductory quest to open the zone. There was no one else there but me and I did not loot it off a mob.

Just lying on the ground! Later I realized it was highlighted on my mini-map but I spotted it with my naked eyes.

Glad to know that this still is spawning off of Apocalypse. I had it spawn for me while doing the Shadowlands intro quests in The Maw, in front of the cave that Jaina has us hide in.

I already had the appearance unlocked from a different DK, but atleast I know my Worgen DK can go into the secret room if I want to.

Level 53 kicking around in Maldraxus (59.75, 74.76 - near where you bring back the ingredients for mixing) and this dropped from random mob. Came here, learned about the unique appearance (Bonereaper's Hook) and decided to get it.

I first did the Legion intro quest chain to get the Dalaran hearthstone, and as part of that picked up the quest to get Apocalypse. I've just downed Prof Putrecide and will probably finish ICC (on 10 man heroic if anyone is interested) before heading off to get Apocalypse.

This thing just does not want to drop.

I've completed the DK campaign, gotten all the weapons, and spent hours in proving grounds spamming army of the dead and apocalypse with the actual weapon in use.

Am I missing something?

Something interesting i found out today: I have 3 DKs in my account (or more) and i am playing my newish DK (Which have not even done the Legion introduction), and the book dropped today in Torghast on this fresh DK.

So apparently you can get it on alts after getting the legion introduction on another DK in your account :D AND the book can drop even if you already have the hidden skin

There was a Death Knight that said "why have you summoned me" or something and dropped the book in the middle of the map.
I'm glad went to read it while being killed lol.
